Follow These Steps To Request A PSP

First, each employer requesting a PSP must complete and return a PSP customer agreement to Safety On Site, Inc.  This agreement must be on file with us before we can meet your request. You only need to complete and return this form the first time you request a PSP.

Second, you need to provide information on your prospective driver.  You will need to complete this for each driver, each time you make a request.

Finally, you must obtain permission of the applicant before requesting a PSP. Have each applicant complete the Applicant Disclosure and Authorization to Obtain PSP form below. You must use this form to obtain permission.

Please scan and save these documents to your computer.  You may then submit each document to Safety On Site below.  This is the preferred way to submit PSP documents.  Alternately, you may submit these documents to us by fax at 888-233-2006, or email them to support@safety-on-site.com.

You must keep a copy of these forms on file for 5 years regardless of your hiring decision. Safety On Site will invoice your account for each report requested.
